Coordinator with Phoenix Mental Health & Wellness and Northlight
Counseling Associates, you will be responsible for providing
support to patients and the clinical staff. The Associate Patient
Care Coordinator will greet and support the check in and check out
for patients, register new patients, prepare paperwork and review
for accuracy, verify completion of consents and insurance
eligibility, process payments for in-person appointments, handle
existing patient communications, manage internal referrals and
ensure work locations are presentable and welcoming. In 2025,
Phoenix Mental Health and Wellness will be partnering with
Northlight Counseling Associates to become Elevate Mental Health.
Together, Elevate Mental Health will proudly provide comprehensive
medication management and counseling services, both virtually and
in-office, across four locations in Arizona. Our psychiatric care
services focus on evaluating, diagnosing, and treating a wide range
of mental health conditions. Location: 3839 N. 3rd St. (Suite 104),
Phoenix, AZ Schedule: Monday-Friday, 8am-5pm PST Primary
Responsibilities: - Review each patient’s chart prior to
appointment and ensure consents and insurance verification is
complete - Complete patients check in and check out; obtain
registration forms, collect and upload insurance cards and
identification, verify completion of consents and confirm insurance
eligibility - Assist with onboarding new patients, help patient’s
access patient portal, prepare and confirm completion of paperwork,
ensure consents and insurance verification are complete - Process
payments or co-pays, deductibles, co-insurance, self-pay, and
missed appointment fees - Assist in update and/or scheduling of
existing patients, including assisting with internal referrals
initiated by the provider - Answer phone calls and return
voicemails; record all requests in the EHR system for Physician
follow-up, including refill requests - Support prescribers in
filing prior authorizations for insurance and submitting and
monitoring lab tests and results - Order and restock supplies -
Monitor clinician/admin/OBC inbox and respond accordingly and take
necessary steps - Ensure all locations are welcoming and adhere to
brand guidelines - Secure patient information and maintain patient
confidentiality Patient Care Duties: - Obtain and record: vital
signs, weight, medication profile, allergies, chief complaint,
signature/initials where appropriate before appointment with
Psychiatrist - Gather patient history, collateral documentation and
updates for patient chart - Administer screening tools What are the

Diploma / GED (or higher) - 1 years of experience in an office
environment (preferably in a mental health or medical setting)
Preferred Qualifications: - Experience with insurance verification
- Experience working with computers for professional communication
and medical documentation - Microsoft Excel, Outlook, Athena RMS
(or other EHRs) - Basic understanding of medical terminology -
Ability to interpret a variety of data and instructions furnished
in written, oral, diagram, or schedule form - Written and verbal
communication skills to explain sensitive information clearly and
professionally to diverse audiences, including non-medical staff -
Ability to work independently, as well as be part of the team,
including accomplishing multiple tasks in an environment with
interruptions Pay is based on several factors including but not
limited to local labor markets, education, work experience,
