Independent Contract Physician position. GENERAL PRIORITIES Perform
vasectomy procedures. Supervise post-procedure care until all
patients are stable and/or discharged as defined by protocol. Order
post-procedure testing, including post-vasectomy semen analysis.
Participate in the review and management of post-vasectomy semen
analysis results. Comply with all informed consent, mandated
waiting periods, and parental consent notification laws. Document
compliance with all laws. Ensure consents are properly signed and
in order prior to the performance of the procedure. Obtain a
pre-procedure history, physical examination, and appropriate
laboratory tests as indicated. Qualifications EDUCATION AND/OR
EXPERIENCE Doctor of Medicine - Certified MD or DO from an
accredited medical or osteopathic school. Licensed to practice
medicine in Arizona. BLS certification. Privileged to perform
vasectomies based on prior experience and/or training.
