Eligible Family or Internal Medicine Physician to join our team in
Queen Creek, AZ. If you're passionate about delivering exceptional

patient and provider wellness . You will play a pivotal role in
transforming healthcare delivery through a collaborative team
approach with our population team health. Our clinic is currently
focused on primary care services with chronic disease management,
Medicare wellness, physicals, well-child examinations, acute care
visits and minor in-office procedures. Position Highlights:
Outpatient setting focused on clinical excellence and patient
satisfaction within a value-based model Emphasize quality outcomes
through collaborative care with our population health team Monday
through Friday, with occasional Saturday Light call: phone call
consult only 36 hours of patient contact; 4 hours admin time
Support includes Medical Assistants, integrated team of
specialists, Pharm D, onsite lab, and additional resources We seek
candidates who: Are Board Certified or Board Eligible in Family
Medicine or Internal Medicine Prefers to see adult patient
population Have experience with or strong interest in value-based
care, quality improvement, and population health Are
patient-focused, friendly, and willing to collaborate in a team
